Abstract:Objective To explore the methods of radiofrequency catheter ablation (RFCA) for atrioventricular reentrant tachycardia(AVRT) closing to His buddle accessory pathways. Methods The right-side accessory atrioventricular pathways near His buddle were verified by electrophysiology stimulate, and the ablative target were measured by left oblique projection 45 degree, with the shortest A-V and V-A interval or complete fusion of AV or VA around tricuspid ring near His buddle. The successful ablative target always showed a very small H wave which was much smaller than that of His buddle electrogram. It was emphasized that RFCA was performed at sinus rhythm or supraventricular tachycardia with little energy at initial stage. It can increase the power and prolong the discharge time under effective conditions. Results RFCA was conducted successfully on 15 patients without serious complication. Conclusion RFCA is a safe and effective method for the therapy of His buddle accessory pathway under accurate mapping and ablation.
