Abstract:Objective To explore the value of electrocardiogram QRS wave duration and QTc interval dynamic monitoring in the evaluation of prognosis of hospitalized patients with acute heart failure (AHF).Methods The clinical data of 104 hospitalized patients with AHF were retrospectively analyzed. According to the prognosis based on the Nterminal pro brain natriuretic peptide level(NT-Pro BNP)72h after treatment, the patients were divided into the fine group (group A, n=51), the poor group (group B, n=40) and the death group (group C, n=13). The differences in NT-Pro BNP level, electrocardiogram QRS wave duration and QTc interval of patients admitted to hospital immediately (T1), consulting for 24h (T2),consulting for 72h (T3) and at other time points were retrospectively analyzed. The correlation between QRS wave duration , QTc interval and NT-Pro BNP level was analyzed. Results ① In T1-T3, it was showed that the NT-Pro BNP levels of group B and C were respectively higher than that of group A (P<0.05). Their difference was of statistical significance; While the difference between group B and C was of no statistical significance(P>0.05). In T2-T3, the monitoring results of above indicators were significantly lower than that in T1(P<0.05); The fall range of group B and C was respectively lower than that of group A with a statistical significance(P<0.05); While there was no statistically significant difference between group B and C(P>0.05);② In T1-T3, QRS wave duration, QTc interval and NT-Pro BNP level were significantly correlated(P<0.05). Conclusion Electrocardiogram QRS wave duration and QTc interval dynamic monitoring can be used as an effective method for the prognosis of hospitalized patients with AHF to improve the pertinence and effectiveness of the followup treatment and the prognosis of patients.
