Abstract:Objective To classify the RR-Lorenz plot with discontinuous scatter spots in the 45° line and explore the corresponding ECG diagnosis. MethodsAccording to the distance of discontinuous scatter spots from the origin of coordinates, times of discontinuity and whether being shaped in a square, the Lorenz plots are divided into 5 types, which are referred as near-the-origin discontinuous type, intermediate discontinuous type, continuous incomplete type, twice interrupting type and square-shaped discontinuous type, respectively. The RR-Lorenz plot was manually drawn by the RR intervals of ECG, till the outline of the 5 types was done. ResultsIn the enrolled 31 cases, 13 cases were near-the-origin discontinuous typed, accounting for 41.9%. It was caused by tachyarrhythmia such as paroxysmal supraventricular tachycardia, paroxysmal atrial flutter, interpolated premature ventricular contraction, paroxysmal ventricular tachycardia, paroxysmal auricular tachycardia and so on that discontinuous scatter spots appeared on ECG. Nine cases were intermediate discontinuous typed whose ECGs represented as seconddegree atrioventricular block or second degree sino-atrial block. Four cases were continuous-incomplete typed caused by atrionector functions separation lengthways. Two cases were twice interrupting typed with ECG shaped in a combination of near-the-origin discontinuous type and intermediate discontinuous type. Three cases were square-shaped discontinuous typed whose ECG represented as sinus rhythm or auricular fibrillation accompanied by escaped rhythm, or a combination of discontinuous pacemaker ECG and twice interrupting typed ECG or near-the-origin discontinuous typed one. ConclusionThe RRLorenz plot of second degree atrioventricular block or sinoatrial block is in the intermediate discontinuous type. If twice interrupting type is found among scatter spots in the 45° line, then the patients suffer not only bradyarrhythmia but also tachyarrhythmia. The electrophysiologic concept of atrionector functions separation lengthways is newly put forward based on RR-Lorenz plot. It reveals the value of RR-Lorenz plot in exploring, investigating and analyzing more and more physiological and pathological phenomena on heart.
