Abstract: Objective To explore the relationship between serum creatine kinase isoenzyme (CK-MB), copeptin, lactate dehydrogenase (LDH) and the severity of patients with chronic heart failure (HF). Methods A total of 132 patients with chronic HF were retrospectively included as the study group, and they were further divided into severe, moderate and mild HF subgroups according to the results of 6-minute walk test (6MWT) at admission. Another 132 healthy physical examinees were enrolled as the control group. The levels of serum CK-MB, copeptin and LDH, and echocardiographic indicators [left ventricular end diastolic diameter (LVEDD), LVEF and left ventricular end systolic diameter (LVESD)] were compared separately between the two groups and patients with different severities of HF at admission; the correlation between each serum indicator level and echocardiographic indicators at admission were analyzed. ROC curve was used to analyze the efficacy of each serum indicator in predicting severe HF at admission. Results The levels of serum CK-MB, copeptin and LDH, and LVEDD and LVESD in the study group were all higher than those in the control group while LVEF was lower (P<0.05). Serum CK-MB, copeptin and LDH were positively correlated with LVEDD and LVESD scores, while these indicators were negatively correlated with LVEF (P<0.05). The levels of serum CK-MB, copeptin and LDH, and cardiac function indicators of LVEDD and LVESD in the mild HF subgroup were all significantly lower than those in the moderate and severe HF subgroups while LVEF was significantly higher than that in the moderate and severe HF subgroups (P<0.05). Serum CK-MB, copeptin and LDH were all positively correlated with 6MWT (P<0.05). The AUC value of the combination of serum CK-MB, copeptin and LDH levels at admission was 0.858 in the prediction of severe HF. Conclusions The levels of serum CK-MB, copeptin and LDH are closely related to the severity of HF. Clinical detection of their levels is of great value in evaluating the severity of these patients and carrying out subsequent clinical treatment.
