Clinical electrocardiology analysis of patients with idiopathic premature ventricular contraction or ventricular tachycardia originating from the aortic root
Abstract:Objective To explore the surface electrocardiogram (ECG) and intracardiac electrophysiological characteristics of patients with idiopathic premature ventricular contraction (IPVC) or ventricular tachycardia (VT) originating from the aortic root. Methods Retrospective analysis was performed on the clinical data of 32 patients diagnosed with IPVC or VT originating from the aortic root (research group) in our hospital. Their surface ECG and intracardiac electrophysiological characteristics were analyzed. Thirty patients with right ventricular outflow tract PVC or VT who had been treated in our hospital during the same period were selected as the control group. The ECGrelated parameters were compared between the two groups, and their diagnostic efficacy on PVC or VT originating from the aortic root was analyzed by using ROC curve. ResultsPatients in the research group are mainly manifested by left bundle branch block plus downward electrical axis with prethoracic lead shifting before lead V3, primarily rsr~ and rS waveform in lead Ⅰ, and mainly QS waveform in lead aVL. The R wave duration and R wave amplitude index of patients in the research group are both higher than those in the control group (P>0.05). ROC curve analysis shows that ECGrelated parameters have certain diagnostic efficacy on the patients with PVC or VT originating from the aortic root (P<0.05). Conclusion Compared with ECG results of IPVC or VT originating from common sites, those originating from the aortic root are featured by unique ECG manifestations. R wave duration and R wave amplitude index have certain clinical values on distinguishing IPVC or VT originating from the aortic root from right ventricular outflow tract IPVC or VT.
