229 Diagnostic value of electrophysiology and electrocardiographic QRS scoring in patients with cardiac amyloidosis

Cardiac amyloidosis (CA) is a kind of relatively rare infiltrative cardiomyopathies with poor clinical prognosis, and it is difficult to be diagnosed at an early phase. Therefore, it is clinically significant to explore diagnostic methods for the early diagnosis and early treatment of CA. This paper introduces the electrophysiological mechanism and electrocardiographic features of CA, and the diagnostic value of electrocardiographic QRS scoring on CA, in order to improve its  diagnostic rate at an early phase.

234 Expert consensus on ECG identification applied in the insurance industry
With biological identification features of universality, portability, uniqueness and stability, electrocardiogram (ECG) as a novel identification method has crossover application in the information field. It enriches and improves the existing biometric identification techniques, and is suitable for customer  identification in the insurance industry. This paper firstly introduces common biometric identification techniques used in the insurance industry, and ECG biometric identification technique and its advantages. It then explains the main content of ECG biometric identification technique, and systematically reviews the latest research advances at home and abroad related to the characteristic points of ECG identification and their extraction. Based on the application practice of ECG identification technique in the domestic insurance industry for nearly 20 years, this paper outlines its technical identification process. Finally, this paper looks forward to the development of the technique in the future. It is aimed to further promote the application of this technique in the insurance industry and other industries.

243 Methodology of QT interval measurement guided by ICH-E14 Guidelines

Significant prolongation of QT interval would easily cause malignant arrhythmias, and therefore in the research and development of new drugs, great importance should be attached to the evaluation of QT interval prolongation. As a necessary guiding principle for new drug research and development, ICH-E14 Guidelines, in the form of industry norms, stipulates that new non-antiarrhythmic drugs must be evaluated for QT/QTc interval prolongation before marketing. It means that electrocardiogram (ECG) monitoring will play an important role in the safety evaluation of this kind of drugs. In order to complete the collection and comprehensive analysis of ECG data with high standards and quality, this paper focuses on the methodology of QT/QTc interval measurement from several aspects. It introduces the requirements for instruments and personnel during QT interval measurement, the selection of measurement leads, the determination of QT interval starting and end points,  QT interval measurement under different circumstances,different correction methods for QT intervals, and notes during data measurement.

248 Research advances in effects of non-antiarrhythmic drugs on QT/QTc interval

The National Medical Products Administration demands that the relevant requirements for  drug clinical studies initiated since July 31, 2023 follow the “E14: the clinical evaluation of QT/QTc interval prolongation and proarrhythmic potential for non-antiarrhythmic drugs”. This paper introduces QT interval and QTc, non-antiarrhythmic drugs potentially leading to QT interval prolongation and even torsade de pointes, the mechanism of QT interval prolongation, predisposing factors, and other related issues. It is aimed to arouse more attention to the effects of new nonantiarrhythmic drugs on QT/QTc interval among clinicians, and to improve the safety of drug use.

254 Research progress of leadless pacemaker

Leadless pacemakers (LPMs), which have emerged in recent years, avoid the lead and pocketrelated complications associated with conventional lead pacemakers, such as tricuspid regurgitation, lead fracture, lead endocarditis or pocket infection. Not only has LPM a significant advantage over conventional pacemakers in terms of safety, but also its stability of pacingrelated parameters has been demonstrated during a relatively long followup period.  At present, LPM has been approved for marketing in China. Although the application of LPM is still in its infancy, the implantation volume is growing rapidly, therefore its development prospect in the future is good. This article reviews  the clinical application evidences of existing LPM, related techniques and postoperative management of LPM implantation, future development direction of LPM, and so on.

261 Prevention of sudden death in patients with arrhythmogenic right ventricular cardiomyopathy: Implantable cardioverter-defibrillator or catheter ablation?

Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a kind of genetic cardiomyopathy in which the normal myocardium is replaced by fat and (or) fibrous tissue. The lesion is mainly confined to the right ventricle, and can also involve the left ventricle, with clinical manifestations of heart failure, ventricular tachycardia and sudden death. At present, treatments of ARVC mainly include exercise restrictions, application of antiarrhythmic drugs, implantation of implantable cardioverterdefibrillator(ICD) and catheter ablation. This review mainly summarizes the latest research development of ICD and catheter ablation in the treatment of ARVC and prevention of sudden death in ARVC patients.

265 Comparison  of diagnostic value on supraventricular tachycardia  between transesophageal electrophysiologic study and intracardiac electrophysiologic study

Objective  To investigate the value of transesophageal electrophysiologic study (TEEPS) and intracardiac electrophysiological study (IEPS) in diagnosing supraventricular tachycardia (SVT). Methods  IEPS was performed in 197 patients who had undergone TEEPS to induce and terminate SVT. The results were comparatively analyzed. Results IEPS diagnosis results are considered as the gold criteria for SVT, and applied to verify the diagnostic accuracy of TEEPS. By TEEPS, the diagnostic accuracy rate of atrioventricular nodal reentrant tachycardia (AVNRT) is 97.1%, the diagnostic accuracy rate of atrioventricular reentrant tachycardia (AVRT) is 94.1%, and  the diagnostic accuracy rate of atrial tachycardia (AT) is 100%. The detection rate of AVNRT by TEEPS and IEPS are separately 73.2% and 72.3% while the detection rate of AVRT by TEEPS and IEPS are both 26.7%, without statistically significant differences (P>0.05). Spontaneous or stimulation induced tachycardia during TEEPS could be terminated by transesophageal atrial pacing except for AT. Conclusion  TEEPS has a relatively high application value in the determination of SVT mechanism, location of atrioventricular accessory bypass and  termination of tachycardia. As a routine screening tool before radiofrequency ablation, TEEPS is capable of shortening operation time, and reduce operation trauma and economic burden.

269 Analysis of tent-shaped T wave characteristics in 63 patients with chronic kidney disease without hyperkalemia

Objective  To analyze the characteristics of tent-shaped T wave in patients with chronic kidney disease(CKD) in a non-hyperkalemia state. Methods  We selected 63 hyperkalemia inpatients diagnosed with tent-shaped T wave by routine body surface ECG (hyperkalemia group), and another 63 CKD inpatients without hyperkalemia (hyperkalemia memory group). The duration and amplitude of T wave in lead V3 were measured, and compared between the two groups. Results  In the two groups, the T wave duration in lead V3 is separately (146.10±20.47) ms and (153.44±16.99) ms while the amplitude of T wave is 1.50(1.00, 2.00) mV and 1.10(0.70, 1.50) mV, respectively, both with statistically significant  differences (P<0.05). Conclusion  The morphological changes of tentshaped T wave are related to serum potassium concentration. The hyperkalemia memory phenomenon may be associated with the abnormal distribution of potassium ions in local myocardium, or the dynamic balance process of serum potassium concentration.

272 Analysis of abnormal ECG characteristics in physical examination of 15 225 employees working in enterprises and institutions

Objective  To investigate the detection situation and characteristics of abnormal ECGs of on-post staff and workers in enterprises and institutions during physical examination, and to provide references for their preventive health care. Methods  We selected ECG data of 15 225 onpost staff and workers from Xiangcheng District, Suzhou  who had undergone routine physical examination in 2022. Descriptive statistical analysis was performed on the population distribution characteristics and clinical features of abnormal ECGs by different sexes and ages. Results  The overall detection rate of abnormal ECGs is 31.55%, mainly including sinus bradycardia (9.07%), ST-T changes (5.96%), right axis deviation (4.43%), left ventricular high voltage (2.58%), sinus tachycardia (2.40%), left axis deviation (1.89%), complete right bundle branch block (CRBBB, 1.66%), and nonspecific intraventricular block (1.25%). The detection rate of abnormal ECGs in males is higher than that in females, with statistically significant difference (P<0.05). The detection rate of abnormal ECGs in the 51-60 age group is higher than that in other age groups of the same sex (P<0.05); there is no statistically significant difference of detection rate of abnormal ECGs between males and females of the 51-60 age group (P>0.05).  The detection rate of single abnormal ECG index shows significant difference between ages or sexes.  Right axis deviation, sinus tachycardia, and short PR interval are more common in the 21-30 age group while sinus bradycardia, ST-T changes,  left ventricular high voltage,left axis deviation,   CRBBB, premature beats, first degree atrioventricular block,  and left anterior fascicular block are more common in the 51-60 age group. In the females, the detection rates of ST-T changes, sinus tachycardia, and accelerated atrial escape rhythm are significantly higher than those in the males while in the males, the detection rates of sinus bradycardia, left ventricular high voltage, left axis deviation, CRBBB, nonspecific intraventricular block,first degree atrioventricular block, and prolonged P-wave duration  are significantly higher than those in the females (P<0.05). Conclusion The detection rate of abnormal ECGs is relatively high among onpost staff and workers in enterprises and institutions, which is closely related to age and sex. Regular physical examination and targeted health management help to prevent cardiovascular diseases among this kind of population.

277 Characteristics and mathematical principles of scatter plot of ventricular parasystole  with double and triple rhythm

We selected typical cases diagnosed with ventricular parasystole according to ambulatory electrocardiography, and summarized the characteristics of Lorenz scatter plot and difference scatter plot of ventricular parasystole with double and triple rhythm. By using mathematical principles, we analyzed and calculated the linear equations of   the premature ventricular contraction point set (NVN) and the double rhythm point set (VNV) in the Lorenz scatter plot, and the double rhythm related point sets (NVNV and VNVN) and the characteristic point set of triple rhythm (VNNV) in the difference scatter plot. The mathematical model was constructed by the Geometer Sketchpad to provide reliable basis for rapid and accurate clinical diagnosis.

283 Pathogenesis and related treatment of pulmonary fibrosis after novel coronavirus infection

Novel coronavirus infection is a highly infectious respiratory disease, which mainly affects the lungs. The progression of this disease is probably related to the excessive release of inflammatory mediators (such as cytokines), which further causes lung deterioration resulting in acute respiratory distress syndrome and respiratory failure, seriously threatening patients’lives. Pulmonary fibrosis is a longterm pulmonary sequela faced by the survivors of critical illness from novel coronavirus infection. It is also one of important risk factors leading to the aggravation of corona virus disease 2019. This article summarizes the research advances in the pathogenesis and treatment of pulmonary fibrosis after novel coronavirus infection.

289 Progress in molecular genetics of sudden cardiac death

The etiology of sudden unexplained death (SUD) is complicated, and its  incidence remains significant at present. The key to its prevention and treatment lies in identifying the underlying pathogenic factors. With the progress of molecular biology techniques, numerous  studies at home and abroad have substantiated a strong association between sudden cardiac death (SCD) caused by pathogenic gene mutations and SUD. The conspicuous clustering characteristics observed in cases of Yunnan sudden unexplained death (YNSUD) indicate the potential involvement of molecular genetics. In the population of the affected area, the detection rates of abnormal electrocardiogram (ECG) indicators increased while cardiac pathological changes were found in 90% of sudden death cases, which further implies a possible correlation between YNSUD and pathogenic gene mutations related to SCD. Understanding the genetic predisposition, genetic mutation, risk factors, and pathogenic mechanisms of SCD contributes to identifying the highrisk factors associated with this kind of sudden death, so as to carry out targeted prevention and treatment. This paper reviews the current research trends in hereditary arrhythmias and hereditary cardiomyopathy related to SUD, and summarizes the latest research results and pathogenic factors.It is aimed to provide a foundation for further investigations on the etiology.
