229 Analysis of ST-segment depression

ST-segment change is an ECG manifestation which is commonly seen in clinical practice and easily misdiagnosed. The reason is that ST-segment is mainly descriptive in ECG report and it is difficult to measure the end of S wave and the beginning of T wave accurately. All of -T changes are not caused by myocardial ischemia since many factors contribute to ST-T changes. It has vital clinical significance to accurately identify predictive ST-T changes for ECG's important diagnostic value on myocardial ischemia. This paper reviews the related basic concepts, generalizes four determining methods of ST-segment depression, summarizes its correspondence analysis and several common ST-depression of special types, analyzes its prognosis and risk stratification, and explains the mechanism, so as to provide concise thinking of analysis and diagnosis for more clinicians.

234 Epicardial mapping and ablation for ventricular tachycardia:techniques to optimize success and minimize complications

Epicardial mapping provides essential information to better understand the threedimensionality of ventricular tachycardia circuits. Once epicardial access is obtained, mapping techniques such as multipolar mapping, isochronal late activation mapping and changing wavefronts can help determine essential regions for ablation, while limiting the amount of ablation required. However, successful epicardial mapping and ablation requires clear anatomic understanding to maximize success and minimize complications.

247 Comparison of substrate and outcomes for ventricular tachycardia ablation between the patients with ischemic and nonischemic cardiomyopathy

Supported by the experience and data from the Southlake in Canada and other centers, this paper compares the outcomes for ventricular tachycardia ablation between the patients with ischemic cardiomyopathy(ICM) and nonischemic cardiomyopathy(NICM). It implies that the outcomes are better for substratebased ablation in ICM patients compared with the outcomes in NICM patients. However, it is challenging to identify NICM substrates. Longterm outcomes for VT ablation remain worse in NICM patients than those in ICM patients. But if the reduction of VT burden is regarded as a main target rather than complete cure, the outcomes are still acceptable.

256 The automatic analysis technology of ambulatory electrocardiography

By longtime monitoring, ambulatory electrocardiography(AECG) is irreplaceable for recording and diagnosing transient and intermittent ECG abnormalities especially arrhythmias. The large amount of data based on the longtime monitoring of AECG can be processed preliminarily via the automatic analysis technology efficiently and accurately. This paper focuses on three essential parts of AECG automatic analysis—calculation of the heart rate, identification of arrhythmias and measurement of STsegment deviation.

260 Design and application of electrocardiogram for children

As a special group, children especially infants have their own anatomic and physiologic features of cardiovascular system, and frequencies of cardiac electrical activities, which is significantly different from adults. The collection method, analysis parameters and diagnostic criteria for children electrocardiogram(ECG) are also different from those for adults. However, the ECG equipments and diagnostic criteria for children are almost the same as those for adults at present. Therefore, it is urgent to develop a professional ECG machine for children, which is capable of collecting childrens ECG signals nondestructively and supporting children ECG waveform algorithm.

267 Evaluation of domestic ECG machine on the accuracy of data acquisition and automatic analysis

ObjectiveTo explore the equivalence of performance between domestic and imported ECG machines. [WTHZ]MethodsOne hundred subjects were randomly divided into group A and B, each with 50 cases. In group A, routine 12lead ECG was recorded simultaneously by ECG machine produced by Wuhan Zoncare Company(type 1) and ECG machine produced by General Electric Company(type 2). In group B, routine 12lead ECG was recorded simultaneously by type 1 ECG machine and ECG machine produced by Philips Electronics(type 3). The three types of ECG machines were set the same collection parameters, connected the lead wires of different types of machines with three links, and collected the routine 12lead ECG signals of the same subject simultaneously and synchronously. [WTHZ]ResultsEight parameters and the amplitude of each waveform automatically measured by the equipments were analyzed by three statistical methods. Analysis results all confirmed that there was no statistical difference between the data separately obtained by domestic and imported ECG machines. It demonstrated good consistency and correlation of the data. [WTHZ]ConclusionThe objective data supports the equivalence of performance between domestic and imported ECG machines. And our fellow countrymen can utilize the domestic ones with confidence.

271 Correlation analysis of sleep apnea monitoring and heart rate variability by ambulatory electrocardiography

Objective  To investigate the change regulation of heart rate variability(HRV) in patients with abnormalities in sleep apnea monitoring, and thus to evaluate the cardiovascular function and predict the incidence of cardiovascular events. Methods  A total of 136 patients with different degrees of sleep snoring were synchronously carried out polysomnography(PSG) testing and ambulatory electrocardiography(AECG) monitoring. Ninetyseven ones diagnosed as having sleep apnea syndrome(SAS) were screened out of those patients, namely SAS group, and then were divided into three subgroups—mild, moderate and severe subgroups according to apnea hypopnea index. The other 39 patients were served as control group. With the help of the second generation of SAS screening technology by DMS company, we analyzed the monitored respiratory waveforms, time domain HRV indexes(SDNN, rMSSD, PNN50) and frequency domain HRV indexes(LF, HF). Results  The 24hour SDNN, rMSSD, PNN50 and HF in the SAS group were all significantly lower than those in the control group, with statistically significant differences(P<0.05). LF values of patients in the moderate and severe SAS subgroups in sleep were both significantly higher than those in the control group, with statistically significant differences(P<0.05). Conclusion  The severer SAS detected by PSG and AECG is, the lower the values of SDNN, rMSSD, PNN50 and HF indexes will be, bringing more significant hypofunction of vagus nerve and higher risk of cardiovascular events caused by increased sympathetic nerve tension.

275 Curative effect of wenxin granule combined with metoprolol on patients with hypertensive heart disease complicating premature ventricular contraction

Objective  To explore the curative effect of wenxin granule combined with metoprolol on patients with hypertensive heart disease complicating premature ventricular contraction(PVC). Methods  Seventytwo patients diagnosed with hypertensive heart disease complicating PVC in recent years were divided into two groups. Conventional therapy and oral medication of metoprolol were adopted by the control group while the observation group was given additional treatment of wenxin granule besides therapies of the control group. The curative effect was observed in the two groups and clinical experience was summarized. Results  There were statistically significant differences in the total effective rate, control time and control rate of PVC, treatment compliance and incidence of cardiovascular events between the two groups(P<0.05). The difference of the rate of adverse reactions was not statistically significant between the two groups (P>0.05). Conclusion  For the patients with hypertensive heart disease complicating PVC, the combined therapy of wenxin granule and metoprolol can control PVC rapidly and effectively, improve therapy compliance, reduce the incidence of cardiovascular events, and decrease the rate of adverse reactions.

278 Changes and clinical significance of deceleration capacity of rate in patients with paroxysmal atrial fibrillation

Objective  To investigate the changes and clinical significance of deceleration capacity of rate(DC) in patients with paroxysmal atrial fibrillation(PAF).Methods  Ninety patients diagnosed with PAF(PAF group) were selected from the inpatients in our hospital from January 2015 to October 2016 while 133 healthy subjects were selected as control group. They underwent 24hour ambulatory electrocardiography and echocardiography examinations. Between the two groups, their DC, acceleration capacity of heart rate(AC), heart rate variability(HRV), left atrial diameter(LAD) and left ventricular ejection fraction(LVEF) were compared and analyzed. [WTHZ]Results(ⅰ) Compared with the control group, DC value, absolute value of AC, SDNN, rMSSD and LAD all significantly increased in PAF group(P<0.05) while the 24hour average heart rate and LF/HF both significantly decreased(P<0.05). (ⅱ) There was no significant difference of DC value between males and females in either group. However, if compared with the same sex in the control group, DC value of males in PAF group significantly increased(P<0.05) while the value showed an increasing trend in females. (ⅲ) Whether in the control group or in PAF group, DC value was negatively correlated with age(r<-0.262 vs. r<-0.548,P<0.01). If compared with those in the same age subgroup of the control group, the DC value of the patients younger than 60 years old in PAF group significantly increased(P<0.05) while the value showed an increasing trend in those older than 60 years old. (ⅳ) The LAD value of patients in PAF group was significantly higher than that in control group(P<0.05). However, the DC values in PAF group were both significantly higher than those in control group(P<0.05) whether for those with LAD≤40 mm or those with LAD>40 mm. Conclusion  Increased DC value in PAF patients suggests increased tension of vagus nerve. The index is negatively correlated with age of patients.

284 Comparative study on the results of treadmill exercise test and coronary angiography

Objective  To analyze and compare the diagnostic value between the results of treadmill exercise test(TET) and coronary angiography(CAG) in patients with coronary heart disease(CHD). Methods  One hundred and fifteen suspected CHD patients who had been admitted to the Department of Cardiology in our hospital were selected as research subjects. They underwent TET and CAG examination, and the testing results were made rounded analysis and comparison. Results  The three methods of CAG, ultrasound and myocardial enzyme examination were applied to diagnose the 115 suspected CHD patients. The outcomes were considered as pathological findings, that was, 75 and 40 patients were diagnosed as positive and negative, respectively. By TET results, 50 patients were true positive while 30 were true negative, and the accuracy rate was 69.57%. The difference between TET and CAG testing results was not statistically significantly(P>0.05). The degree of coronary stenosis was related to the occurrence time and duration of STsegment changes in TET. The occurrence time and duration of STsegment depression among patients with mild, moderate and severe coronary artery lesions varied significantly(P<0.05). Conclusion  TET results provides highly valuable reference for CHD diagnosis and thus TET is worthy of being promoted and applied clinically.

288 Application of RomhiltEstes electrocardiographic scoring system: a new perspective

Romhilt-Estes scoring system is generally applied in diagnosing left ventricular hypertrophy(LVH) at the very beginning. Definite and suspected LVHs were defined when RE score=4 and ≥5, respectively. In recent years, it indicates by researchers that the Romhilt-Estes scoring system and its components can predict allcause mortality, sudden cardiac arrest and various cardiovascular diseases. The scoring system is clinically significant for diagnosing and treating cardiovascular diseases in an early phase.
