153 Application value of ECG in acute coronary syndrome

Acute coronary syndrome (ACS) includes ST-segment elevation myocardial infarction (STEMI), non-ST-segment elevation myocardial infarction (NSTEMI) and unstable angina (UA). Firstly, this paper briefly compares the pathogenesis, clinical manifestations, and   levels of myocardial injury biomarker between NSTEMI and UA. Secondly, it introduces the differences of basic pathology and ECG between STEMI and NSTEMI. This paper focuses on the clinical value of ECG examination on  localization diagnosis of myocardial infarction and chest pain. Finally the review discusses drug treatment and revascularization for patients with ACS.

157 Progress in construction of ECG network information system

ECG network information system can make early identification, realtime monitoring, critical value warning, diagnosis and rehabilitation management of cardiovascular diseases. The construction of ECG network information system provides a fast track for the expansion of quality medical resources and the sinking of expert resources. This review summarizes the development history and present situation of ECG network information system construction, raises common problems in the construction and application, and looks forward to its development in the future.

162  One-hundred-year-old electrocardiology blossoms new brilliance: Progress and prospect of clinical application of electrocardiogram

Since its first clinical application 120 years ago, electrocardiogram (ECG) has still been one of the most important routine examinations in clinical practice. The rapid popularity of portable, remote ECG and cloud ECG platforms accelerates the promotion of ECG technology—from hospitals to the society, family and even everyone. The arrival of body surface three-dimensional electroanatomical mapping visualizes the presentation of abstract ECG morphologies, and clearly reveals the origin of arrhythmia and ECG signal conduction process. This amounts to a “pathological anatomy” like gold-standard validation diagnosis for the ECG. Through deep learning, artificial intelligence (AI)-ECG can explore the fields that cannot be interpreted by ECG before, which greatly expands the connotation of ECG. This paper briefly reviews the development process of ECG technology from remote ECG, three-dimensional ECG and portable ECG to AI-ECG in the past 120 years, and looks forward to its development trend in the future.

162 ECG and heart ultrasound: Seeking a common ground while reserving differences, and complementing each others advantages

Electrocardiogram (ECG) and echocardiography are both routine examinations for heart diseases with unique advantages in the diagnosis and treatment of heart diseases. ECG focuses on the rhythm and frequency of heartbeats, and electrical conduction while echocardiography is good at displaying the structure and morphology of the heart such as the valve, the size of each chamber, the speed and direction of blood flow, and ventricular volume changes during the systolic and diastolic periods of the heart. In clinical practice, the two seek a common ground while  they reserve differences, and complement each others advantages. This article briefly introduces the invention of ECG and echocardiography, focuses on their respective advantages in clinical application, and emphasizes the important role of their complementation in clinical practice.

172  Clinical research progress of treating atrial fibrillation with traditional Chinese medicine

Atrial fibrillation(AF) is one of common arrhythmias in clinical practice. AF gives rise to a series of complications including stroke, thromboembolism and heart failure, which cause high disability rate and mortality, and increased family and social burdens. The treatment technology of AF is becoming more refined in modern medicine while traditional Chinese medicine has also achieved definite effect on AF. This paper reviews the research progress of diagnosing and treating AF with traditional Chinese medicine (TCM) from aspects of knowledge of disease name and pathogeny of AF in TCM, and typical TCM prescriptions, Chinese patent medicine and needle therapy of treating AF in TCM. This paper aims to provide new ideas for the diagnosis and treatment of AF.

177 Comprehensive management of atrial fibrillation rehabilitation

Atrial fibrillation (AF) is one of the most common arrhythmias in clinical practice. Due to its high morbidity, great harm and heavy medical burden, AF has become a major cardiovascular disease that seriously threatens human health. The comprehensive management of AF rehabilitation mainly includes the prevention of stroke, optimization of symptom control, exercise rehabilitation, psychological intervention and  management of risk factors. Compared with traditional treatment, comprehensive management of AF rehabilitation can significantly relieve the  severity of AF patients symptoms and medical burden, and is more conducive to improve clinical prognosis.

183   Relativity between novel coronavirus infection and ECG or heart rate variability

Objective To explore the relationship between the severity of novel coronavirus infection and arrhythmia or heart rate variability (HRV). Methods We selected 178 inpatients and 30 outpatients infected with novel coronavirus. According to the diagnosis results, the inpatients were divided into mild, medium, severe, and critical novel coronavirus infection groups. The baseline data, ECG, and outcomes of the four groups were compared; Logistic regression analysis was performed. HRV indexes were compared between these outpatients and inpatients. Results The incidences of first degree atrioventricular block (Ⅰ° AVB), abnormal Q-wave, and left ventricular high voltage (LVHV) in the critical novel coronavirus infection group are all higher than those in the other three groups while the incidences of Ⅰ° AVB and LVHV in the death group are both higher than those in the good outcome group (P < 0. 05). Univariate Logistic regression analysis shows that age, sex, LVHV and Ⅰ° AVB are related to the severity of novel coronavirus infection. Multivariate regression analysis reveals that age, prolonged QTc and Ⅰ° AVB are independent risk factors for the severity of novel coronavirus infection. The SDNN and trigonometric index of the outpatients are both higher than those of the inpatients (P < 0. 05). Conclusion ECG changes and HRV indexes are related to the severity of novel coronavirus infection, hospitalization rate and clinical outcome.

190 New advances in application of ECG tracking image technology for auxiliary diagnosis of novel coronavirus infection

Since the outbreak of corona virus disease 2019,  deep learning has been widely used in predicting epidemic trends, screening high-risk groups and making early diagnosis. Novel coronavirus infection mainly causes pulmonary diseases, and also affects cardiovascular system, resulting in  electrocardiogram (ECG) abnormalities. ECG tracking image technology is a hot spot in the research of deep learning. Compared with traditional ECG judgment methods, its accuracy and sensitivity are both higher. It can be used in the effective diagnosis, clinical decision-making and prognosis evaluation of patients with novel coronavirus infection. This paper reviews on the ECG changes after novel coronavirus infection, and the advances of ECG tracking image technology applied in the auxiliary diagnosis of novel coronavirus infection, so as to provide references for clinical healthcare workers.

195 Evidence based visualization analysis of literature on  ECG Tp-Te interval in the China Journal Full text Database and Web of Science Database

Objective  To understand the research advances of ECG Tp-Te interval in the past 20 years, and to explore its current research hot spots and future research directions. Methods  From the China Journal Fulltext Database and Web of Science Database, we searched the related literature with “Tp-Te” or “Tp-e” as article search words published in the past 20 years from 2002-01-01 to 2021-12-31. Excel 2019 and bibliometric analysis software VOSviewer 1.6.17 were applied in the literature classification, clustering of key words, and visualization analysis of countries, institutional cooperation and their sequential variation. Maps were correspondingly drawn. Results  A total of 414 Chinese and 669 English papers are retrieved from the two databases, respectively. The number of related Chinese literature has shown an obvious and progressive increase after 2006, reaching the peak in 2017.  Since then, the number of publications has leveled off. Literature in English shows two peaks in all during the statistics period, separately in 2006 and 2021; the number of  literature approximately shows an incremental trend from 2009.   A large number of related articles are published in several fixed journals without significant difference in the number of published articles. In the national clustering diagram of English literature, Turkey, although publishing a large number of papers, has no much cooperation with other countries;  the United States has the  most cooperation with other countries, followed by China. There is no obvious close cooperation between institutions  which published the literature. Key word clustering analysis reveals that the Chinese literature mostly focuses on ventricular arrhythmias. The research topics trend towards chronic heart failure, acute myocardial infarction, acute coronary syndrome, etc. after 2016, and also involve clinical use of drugs. English literature covers long QT interval, Tp-e/QT, mortality, sudden death, Pwave dispersion, obesity, corona virus disease 2019 and so on. Conclusion  Domestic research hot spots related to Tp-Te interval are quite different from those in other countries, which is associated with national conditions, living environment, social status and physical differences.

202 ECG characteristics under the condition of misconnected limb leads

With the growing popularity of ECG network and remote ECG technology, ECG collection personnel is separated from ECG diagnostic physicians. Misconnection of limb leads is not uncommon in clinical practice. The situation gets worse due to inadequate capability and careless omission of ECG collection personnel at work. In this paper, the ECGs of a 37yearold healthy male in 24 kinds of connection ways of limb leads are taken as  practical examples. By analyzing the ECG changes based on Einthoven triangle principle, the ECG characteristics under the condition of misconnected limb leads are summarized, which are provided as references for ECG diagnostic physicians.

208 Pathophysiology and mechanisms of diabetes related arrhythmias

More and more studies have shown that the risk of arrhythmias in diabetes patients is significantly increased, but their mechanisms are complex and have not been fully clarified at present. Based on explaining the relationship between  diabetes and arrhythmias, this article  reviews the mechanisms of diabetes related arrhythmias from aspects of inflammation, oxidative stress, autonomic nervous dysfunction, mitochondrial dysfunction, electrical remodeling, blood glucose levels, glucose fluctuations, etc. It is aimed at providing a novel strategy for the prevention and treatment of this disease.

215 Research advance in clinical application of blood pressure variability

Blood pressure (BP) is one of dynamic physiological variables. Besides BP level itself, abnormal blood pressure variability (BPV) is also a risk factor of cardiovascular disease  in the population at high cardiovascular risk. Abnormal BPV is associated with the occurrence, progression, and severity of damage to target organs such as the heart, brain, kidneys and blood vessels. It will result in increased risks of cardiovascular events, and incidences of cardiovascular and allcause mortalities while  it is also an independent risk factor. In the risk stratification for cardiovascular diseases, besides paying attention to BP level, BPV monitoring should also be a part of routine clinical practice. This article mainly reviews the influence of abnormal BPV on related diseases, and the research advance in its clinical application.
