765 Effect of senate pine spiritual cultivation capsule on heart rate variety and QT dispersion of patients with coronary heart disease
Objective  To discuss the effect of senate pine spiritual cultivation capsule on heart rate variety(HRV) and QT dispersion(QTd) of patients with coronary heart disease(CHD).  MethodsRandomly divide 192 CHD patients into observation group(98 cases) treated with senate pine spiritual cultivation capsule and control group(94 cases), and then observe the changes of indexes related to HRV, QTd and corrected QTd(QTcd) between the two groups respectively before and after the treatment. ResultsAfter the treatment, indexes related to HRV all increased while QTd and QTcd both decreased with a comparatively significant decrease in the observation group. The differences between the two groups were statistically significant(P<0.01). ConclusionIt indicated that senate pine spiritual cultivation capsule could effectively raise indexes related to HRV and reduce QTd, and thus improve the balance of autonomic nerve and prognosis and prevent ventricular arrhythmia.

768 Clinical analysis of arteriosclerosis detector in the prediction of hypertensive disorder complicating pregnancy
Objective  To discuss the value of arteriosclerosis detector utilized in the prediction and prevention of hypertensive disorder complicating pregnancy(HDCP).Methods  With the arteriosclerosis detector manufactured by Colin company in Japan, monitor the brachialankle pulse wave velocity (baPWV) of 500 pregnant women and 100 fertile women.The 225 case of pregnant women with increased baPWV were randomly divided into intervention group(115 cases) and control group(110 cases).Separately compare the incidence of HDCP among the two groups with that of pregnant women with normal baPWV.Results  The number of baPWV increased cases among pregnant women was more than fertile women and the difference was statistically significant(P<0.01).The incidence of HDCP was 7.83% in intervention group, 25.45% in control group and 5.82% among pregnant women with normal baPWV. The incidence of HDCP in intervention group was significantly reduced if compared with that of control group and the difference shows statistical significance(P<0.01).Conclusion  We could apply the arteriosclerosis detector in the prediction of HDCP;The intervention treatment for pregnant women with increased baPWV can reduce the incidence of HDCP.

771 Clinical application and follow up of patients with implantable cardioverter defibrillator
Objective  To report the clinical anti-arrhythmic effectiveness of implantable cardioverter defibrillator (ICD) in 13 cases.Methods  Among the 13 cases, 10 were implanted with unicamerata ICD while 3 with dual-chambered ICD. Make enquirements by telephone and carry out follow-ups in out-patient clinic in order to analyse conditions of the patients and the working state of ICD, through electrocardiogram, dynamic electrocardiogram and the data stored in ICD.Results  Sustainable ventricular tachycardia(VT) or ventricular fibrillation(VF) attacks 38 times in all(sustainable VT 36 times, VF twice), 37 times of which were successfully conversed and yet one time failed.Nonsustainable VT happens 121 times and the attacks all ceased by themselves.The sustained VT was rapid conversed by ICD in ATP(anti-tachycardia pacing) mode for 33 times and low-energy conversion(CV) twice.But once, it progressed to VF during the ICD treatment and was terminated by high-energy defibrillation(DF).The high ventricular rate followed by paroxysmal atrial fibrillation was misidentificated as VT twice in one case and ATP treatments were delivered by ICD. The ventricular arrhythmia storms appeared in one case 3 months after the implantation.Three patients needed to take Amiodarone and (or) Metoprolol because of recurrent attack of VT. The doses should be adjusted according to the severity of arhythmia.Conclusion  To improve the effectiveness of ICD implanted, followed-ups should be strengthened and working parameters should be adjusted in time.Meanwhile, psychological intervention also plays an important role.

774 Analysis of 12 830 cases diagnosed by trans-esophageal atrial pacing
Objective  To discuss the diagnostic value of trans-esophageal atrial pacing (TEAP) for arrhythmias. Methods  During the past 26 years select 12 830 patients who suffered recurrent palpitation, dizziness and brief syncope and accepted TEAP examination. Statistically analyze the results and get knowledge of the occurrences of different arrhythmias. Results  The inducing rate of arrhythmia is 61.23%.The morbidity of supraventricular tachycardia (SVT) is obviously higher than that of ventricular tachycardia (VT) (SVT, 50.35% vs. VT, 1.23%), with a statistically significant difference (P<0.05). By TEAP overdrive suppression, 99.83% of SVT are successfully terminated and 0.15% fail (Patients who require to terminate TEAP don’t be counted), also with a statistically significant difference (P<0.05).No severe complications occur to patients during the examination. Conclusion  It’s one of the most effective non-invasive treatments to utilize TEAP in inducing and terminating VT. The method can provide reliable basis for further clinical treatment, and therefore deserves to be widely promoted and applied.

777 Investigation of the diagnostic standardization of left ventricular hypertrophy by ECG
Objective  To investigate the diagnostic standardization of left ventricular hypertrophy by electrocardiogram(ECG).Methods  With ECG network information system in Yanghu District of the Second People’s Hospital of Changzhou, retrospectively analyse ECG graphic reports from January 2012 to March 2013. Comparing ECG results with corresponding echocardiographic reports, study the diagnostic criteria of left ventricular hypertrophy(including left ventricular pachynsis and/or left ventricular enlargement) by ECG. Results  The study suggests that it’s appropriate to regard left ventricular high voltage with ST-T changes as a necessary condition for the diagnosis of left ventricular hypertrophy(positive rate was 69.39% while coincidence rate was 55.68%). The coincidence rate shows a significant increase if the necessary condition is combined with left axis deviation and/or QRS≥100 ms. Conclusion   Simple left ventricular hypertrophy only represents as left ventricular high voltage with ST-T changes. If left axis deviation is combined with the above representation, the incidence of left ventricular pachynsis and left ventricular enlargement may be equal. If QRS≥100 ms is combined, it mainly implies left ventricular enlargement.

780 Effect of moxifloxacin hydrochloride on QTc in elderly patients with pulmonary infection
Objective  To investigate the effects of moxifloxacin hydrochloride on corrected QT interval (QTc) in elderly patients with pulmonary infection.Methods  A total of 192 elderly patients with pulmonary infection in our hospital were randomly divided into two groups, moxifloxacin hydrochloride group (96 cases) and ceftriaxone sodium group (96 cases), who were separately treated with the corresponding drugs for 10-14 days.Carry out 12-lead simultaneous ECG, blood routine examination and biochemistry complete set before and after the treatment. Results  Comparing with ceftriaxone sodium group, the QTc of moxifloxacin hydrochloride group was prolonged after the treatment, and the difference was statistically significant(P<0.05).Conclusion  Moxifloxacin hydrochloride could prolong QTc interval in elderly patients, which may result in arrhythmia. ECG reexamination should be taken during the treatment.

782 Comparative analysis of hypofunction of left ventricular diastolic function by echocardiogram and dynamic electrocardiogram
Objective   To explore the relationship between the hypofunction of left ventricular diastolic function and the changes of dynamic electrocardiogram(DCG).Methods  Among the patients who have accepted echocardiogram inspections in the physical examination center of our hospital from January 2010 to June 2012, select 58 cases with vestigial left ventricular diastolic function as observation group while randomly choose 40 normal cases inspected by echocardiograms as control group. Carry out 24hour DCG examinations for both of the two groups.Results  Among the observation group, there existed 35 cases(60.3%) with ST-T dynamic changes and 32 cases(55%) with attacks of various arrhythmias. Comparing with the control group, the differences were statistically significant(P<0.05).Conclusion  It hints at the possibility of early performance of heart injury when echocardiogram result indicates the hypofunction of left ventricular diastolic function. However, the heart injury is easy to be ignored and missed clinically, since patients are usually free of any symptom or suffer light ones. For the above reasons, we suggest DCG examination furtherly for patients with vestigial left ventricular diastolic function diagnosed by echocardiograms, in order to clarify the diagnosis and give timely treatment.

784 Clinical study on dynamic electrocardiogram of acute cerebrovascular disease
Objective  To explore the clinical significance of dynamic electrocardiogram(DCG) of patients with acute cerebrovascular disease(ACVD). Methods  Enroll 50 ACVD cases into observation group and another 50 healthy cases into control group. Execute routine electrocardiogram (ECG)and DCG examination upon both of the two groups. For the observation group, compare the abnormity detection rates by ECG with those by DCG. Between the two groups, find out the difference of abnormity detection rates by DCG and analyse the differences of heart rate variability(HRV) time domain indexes(SDNN, SDNAA, rMSSD).Results  For ACVD patients, their abnormity detection rates by DCG were significantly higher than those by ECG and their detection rates of arrhythmia were significantly higher than those of control group. In the aspect of HRV time domain indexes, rMSSD of the observation group was significantly lower than that of the control group while SDNN and SDNAA were lower too.Conclusion  Arrhythmias among ACVD patients usually express as paroxysmal complex arrhythmias including occasional and recurrent atrial premature beat, paroxysmal atrial tachycardia and paroxysmal atrial fibrillation, paroxysmal ventricular heartbeat tachycardia and various atrioventricular blocks. Compared with ECG, DCG could raise the detection rates of paroxysmal, sudden, complex and fatal arrhythmias for ACVD patients. The qualitative analysis of arrhythmia and quantitative analysis of HRV by DCG could provide more comprehensive and reliable information for the diagnosis, evaluation and treatment of heart injury in the attack of ACVD.

787 Analysis of coronary angiographic results of 150 exercise treadmill test positive patients
Objective   To estimate the diagnostic value of exercise treadmill test(TET) for coronary heart disease.Methods   Carry out coronary angiography(CAG) on 150 patients with positive TET results. According to CAG results, divide them into true-positive group and false positive group, and then compare the differences of age and sex between the two groups. Statistically analyse the correlation between the amount of diseased vessels and sex in true-positive group; Compare the difference of leads whose ST-segments shifted between the two groups and analyse the relationship between the TET parameters and CAG.  Results  ① True-positive rate of TET is 60%, with 64.29% in males and 47.37% in females. ② In true-positive group, the proportion of males who are single-vessel, double-vessel and multi-vessel diseased is significantly higher than that of females and the difference is statistically significant(P<0. 05). ③ False-positive rate is high when ST-segments change in inferior leads (P<0. 05). ④ There lie significant differences between the two groups in aspects of total movement time complicated with the amplitude, time of occurrence and lasting time of downward shifting of ST-segments(P<0. 05).Conclusion  TET is an ideal non-invasive testing method to diagnose coronary heart disease. Diagnostic accuracy can be improved if various exercise parameters are combined while comprehensive analysis is carried out.

790 Electrocardiograms of peri-menopausal women: a clinical study of 300 cases
Objective   To discuss the occurrence rate and mechanism of abnormal changes of electrocardiogram (ECG) in peri-menopausal women. Methods   Select 300 copies of ECGs from clinic examination of peri-menopausal women aged from 45 to 55 and carry out 24-hour dynamic electrocardiogram  or take previous data as references if necessary. Divide these women into groups according to menopause, non-menopause and ECG changes, and then make analysis of the formation mechanism and clinical significance of ECG changes. Results   Among 300 cases, the patients with abnormal ECGs accounted for 67.67%, mainly with premature beat, tachycardia and ST-T changes. Higher incidence of heart disease was also found in patients clinically prone to coronary heart disease. Conclusion   Peri-menopausal women had high incidence of ECG abnormality, mainly arrhythmia and ST-T changes.

793 Identification of P waves and assessment of atrial pacing function
In clinical practice recently, the most commonly used heart pacemaker is dual chamber pacemaker in the method of atrioventricular sequential pacing. Atrial pacing becomes an important part of pace-making. All of modern pacemakers have some special functions, including atrial pacing. Dynamic electrocardiogram analysis system should be able to identify P waves in order to assess pacemaker function. This paper demonstrated six cases and showed the value of P-wave-identification in assessing of pacemaker function.

802 Loss of capture caused by ventricular electrode’s micro dislocation: avoid electrode relocation by resetting pacemaker’s parameters
For the pacemaker whose ventricular channel has automatic threshold management function and beat-by-beat monitoring function, if ventricular electrode’s micro dislocation leads to the increase of the pacing threshold, the automatic threshold management function can provide a high-output backup pulse in time when ventricular loss of capture occurs for initial pulse. This function can avoid the risk of ventricular dropped beats and infection of pacemaker pocket due to ventricular electrode’s relocation.

823 The clinical significance of electrocardiogram for coronary heart disease
As a simple and feasible inspection method,electrocardiogram(ECG) has vital significance for the diagnosis and treatment of coronary heart disease(CHD). This paper breifly illustrates the roles ECG plays in the identification of CHD diseased region, infarct-related vascellum, staging and prognosis and also in the instruction treatment scheme of CHD.CHD seriously affects the quality of life of patients, however eary identification and proper treatment will bring benefits for them. With the help of ECG, CHD can be identified at an early stage and given proper treatment clinically.

827 Illustration of electrocardiographic representations when limb lead electrodes are wrongly connected
With the popularity of treadmill graded exercise test and Holter ECG monitoring technology, the probability of mixed wrong connections between upper and lower limb lead wire or left and right hand lead wire of 12 lead simultaneous monitoring ECG increases gradually. Those wrong connection ways are intricate and the graphs are illegible. Taking the limb-lead ECG of a 35-year-old healthy male adult for example, diagrammatize the ECG lead polarity and the tracing order of channels separately in 6 connection ways of lead wire, the right way and 5 wrong ways, especially display the mixed wrong connection ways between the upper and lower limb lead and the ECG representations.
