486 Research on the relationship between blood pressure instant variability and heart rate variability of normal person under respiratory control

ObjectiveTo discuss the relationship between blood pressure instant variability(BPIV) and heart rate variability(HRV) of human under respiratory control. MethodsFiftytwo healthy people′s ECG, blood pressure  (BP)and respiration were synchronously recorded and analyzed when respiratory rate were controlled at 51015 times/min and 81216 times/min. ResultsAt respiratory rate of 1015 times/min and 81216 times/min, RR variation trend and BPBP variation trend were fully synchronized with respiratory wave variation trend. Correlation coefficients(r) of center frequency of RF/HR with peak of respiratory(RSP), BPIV and HRV were 0.924, 0.902 and 0.913, respectively. At respiratory rate of 5 times/min, RR variation trend and BPBP variation trend were desynchronized. ConclusionsAt certain respiratory rate, BPIV is corresponded with the physiological mechanism of respiratory sinus arrhythmia and HRV mechanism caused by respiratory sinus arrhythmia. However, BPIV is not corresponded with the physiological mechanism of HRV mechanism caused by respiratory sinus arrhythmia when respiratory rate is too low.

492 Relevance of circadian rhythm of blood pressure and exercise blood pressure in hypertensives

ObjectiveTo explore the relevance of circadian rhythm of blood pressure and exercise blood pressure in hypertensives. Methods The ambulatory blood pressure and exercise blood pressure of 287 hypertensives were examined. According to the decrease rate of night blood pressure, hypertensives were divided into two groups: normal (dipper) (n=182) and abnormal (nondipper) (n=105) circadian rhythm of blood pressure; According to the blood pressure response to maximal exercise and the recovery rate of blood pressure recovery, hypertensives were divided into two groups: normal (n=202) and abnormal ambulatory blood pressure (n=85). All data between two groups were analysed. Results Nondipper group hypertensives showed higher blood pressure levels than that of dipper group. Compared with the mean blood pressure of 24 h, day and night, abnormal ambulatory blood pressure group were higher than those of normal ambulatory blood pressure group.  ConclusionHypertensives with abnormal circadian rhythm of blood pressure showed abnormal ambulatory blood pressure and higher parameter values of ambulatory blood pressure.

495 Analysis of heart rate variability and clinical prognosis in patients with acute anterior myocardial infarction combined with complete right bundle branch block

ObjectiveTo investigate the characteristics of heart rate variability (HRV) and clinical prognosis in patients with acute anterior myocardial infarction combined with complete right bundle branch block (CRBBB). MethodsA total of 44 cases with acute anterior myocardial infarction combined with CRBBB (observation group) and 225 cases of acute anterior myocardial infarction without CRBBB (control group) were enrolled in the study. Both two groups patients were underwent dynamic electrocardiogram (Holter) examination. HRV values and malignant arrhythmias, coronary disease and prognosis were compared between the two groups. ResultsCompared with control group, observation group had lower levels of HRV values,but higher incidence rates of shock, heart failure, malignant arrhythmia(P<0.01). The results of coronary angiography showed that both two groups patients were present with left anterior descending artery lesions; and there were significant differences between observation group and control group with regard to the incidence rates (82% vs 26%, P<0.01). ConclusionIn patients with acute anterior myocardial infarction with CRBBB, the levels of HRV value were low, the area of myocardial injury was large, and the clinical prognosis was worse.

506 A new teaching experience of electrocardiogram applying threedimension electrophysiological map

ObjectiveTo solve the problem of teaching and studying the electrocardiographic waveform and its theoretic mechanism efficiently. Methods We firstly recommend a new teaching method which based on threedimension electrophysiological mapping and basic mechanism of electrocardiogram (ECG) wave formation. Results The method can well explain almost all waveform characteristics and their theoretic mechanism. Therefore,the new analytic teaching method of ECG is valuable to apply into medical education because it improves students′ ECG interpretation skills. ConclusionIt is a good new teaching method that applying threedimension electrophysiological map into explaining ECG′s waveform and its theoretic mechanism,which was worthy of promotion.
