Abstract Heart rate variability (HRV) is an index which can accurately reflect the activity of cardiac autonomic nervous system. By improving HRV analysis via ambulatory electrocardiographic examination, the sympathetic nerve and vagus nerve tension, and their balance state can be quantitatively evaluated with a combination of time and frequencydomain analysis. Based on the regulatory mechanism of HRV, this review intends to interpret the clinical significance of time and frequencydomain analysis indexes. Combined with recent clinical researches and metaanalysis related to HRV in the fields of coronary atherosclerotic cardiopathy, primary hypertension, heart failure and atrial fibrillation, this paper also reviews the clinical application of HRV testing and analysis in the field of cardiovascular diseases.
