Abstract Athletes are in a special group whose heart makes adaptive changes due to intensive and regular exercise training. Making correct differentiation between physiological ECG changes and pathologically abnormal ECG is related to athletes career, and the prevention and control of sudden death. In recent years, “International recommendations for electrocardiographic interpretation in athletes” has been published in Journal of the American College of Cardiology, British Journal of Sports Medicine and European Heart Journal. Normal/critical/abnormal ECG has been clearly defined in the recommendations, which facilitates the risk evaluation of sudden death before competition in athletes with strong operability. This paper interprets the diagnostic essentials of the above three types of ECGs, briefly introduces the recommended process of risk evaluation of sudden death, and summarized the guidelines unique features different from the previous ECG examination criteria for athletes. It aims to make convenience for the broad masses of ECG clinicians to accurately define and interpret the ECGs of athletes.
