Study on the clinical value of ischemic J wave in patients with acute coronary syndrome

Abstract:Objective To explore the relationship between ischemic J wave in patients with acute coronary syndrome(ACS) and the severity of coronary lesions, analyze the occurrence of cardiovascular events of ischemic J wave in those patients and evaluate their prognosis. Methods According to the existence of the changes of J wave in electrocardiogram(ECG), ACS patients were divided into observation group and control group after coronary angiography(CAG), with 60 cases in each group. The occurrence of arrhythmia indicated by 12 lead ECG and ambulatory electrocardiography,the severity and range of coronary lesions showed by CAG, and ejection fraction measured by two dimensional cardiac ultrasound were retrospectively analyzed among the two groups. Results The incidence of arrhythmia in the observation group was significantly higher than that in the control group(P<0.05). The incidence of multivessel coronary disease in the observation group was also significantly higher than that in the control group(P<0.05). Conclusion For ACS patients, it indicates lesions in left anterior descending branch if ischemic J wave complicating ST segment elevation in front wall lead is observed. Under this circumstance, arrhythmia is prone to happen and ejection fraction is relatively low. Ischemic J wave complicating ST segment depression in inferior wall lead indicates multivessel coronary lesions and poor prognosis.
