661 Advantages of vectorcardiogram in diagnosis of masquerading bundle branch block
Different from left anterior fascicular block(LAFB) complicated with right bundle branch block(RBBB), the lesion degree of cardiac conduction system of masquerading bundle branch block(MBBB) is more serious and usually inclined to progressive deterioration. The diagnosis of MBBB relies on electrocardiogram(ECG), yet yields high omission diagnostic rate. Combing with clinical examples, analyze and discuss the problem based on the essence of bundle branch block as well as the expressive characteristics of vectorcardiogram. When conduction disturbance occurs in some fiber of the left posterior branch, it represents in the vectorcardiogram as dacryon denseness at the middle part of QRS ring while such graphic expression is much more manifest than ECG duration of intrinsicoid deflection (or called ventricular activation time). Therefore, vectorcardiogram could help in clinically discovering MBBB which is hard to be diagnosed by ECG in early phase.

665 The relationship between ages of patients with palpitation and heart rate and atrial arrhythmia
ObjectiveTo get knowledge of the average heart rates and the incidence rates of atrial arrhythmia of patients with palpitation in different ages. Methods1 864 patients with palpitation were enrolled in the study and according to ages, we divided them into young group (≥18 years old, <40 years old) , adult group(≥40 years old, <60 years old)and senior group (≥60 years old, <80 years old), separately 334, 780 and 750 cases fallen in. Carry out transtelephonic electrocardiographic monitoring (TTM) for all patients and analyze the incidence of the average heart rate as well as atrial arrhythmia. ResultsFor the young, adult and senior group separately, the average heart rates were(77.2±10.2)bpm,(74.3±9.3)bpm and(71.0±10.0)bpm(P<0.001)while the incidence rates of atrial arrhythmia were 23.4%, 35.6% and 68.3%(P<0.001). ConclusionWith the increase of the age, the heart rate decreases and the incidence rates of atrial arrhythmia increases.

667 Epidemiological investigation of early repolarization syndrome for the Tibetans in Tibetan regions
ObjectiveTo investigate the epidemiological characteristics of early repolarization syndrome(ERS) in Tibetan population. Methods Collect the basic information and 12 lead ECG data of 8 628 Tibetan college students and postgraduates from different districts and elevations, and count the incidence of ERS and its distribution in different leads of ECG, according to the two types, that was, Jwave and Jpointelevation type separately.ResultsThe general incidence of ERS of the Tibetan people was 13.6%, with higher rate in males than females (P<0.05) . The incidence of Jwavetyped ERS was 4.32% while the other type was 9.31%. ConclusionERS is not rare in natural population, but its incidence is higher in Tibetans, which hints us to consider the relation between the high incidence and their living environment, genetic gene as well as cardiovascular disease.

669 The diagnostic value of 12 lead ambulatory electrocardiogram for silent myocardial ischemia
Objective To investigate the diagnostic value of 12 lead ambulatory electrocardiogram(AECG) for silent myocardial ischemia(SMI).Methods AECGs of 316 patients diagnosed with coronary heart disease were analyzed retrospectively.  Results① The detection rate of STsegment depression by AECG was higher than that of electrocardiogram(ECG), and the difference was statistically significant (P<0.05).② The duration of STsegment depression in SMI was the longest during 06:0012:00, and the difference was statistically significant (P<0.05).③ The attack of SMI was fast frequencydependent, and comparing with the slow frequencydependent SMI, the difference was statistically significant (P<0.05). Conclusion Twelve lead AECG could be used to confirm the frequency, duration and evolution law of silent myocardial ischemia and provides an important basis for clinical diagnosis and prognostic observation of the disease.

672 The clinical significance of exercise tolerance test in the follow up of Kawasaki disease
ObjectiveTo explore the clinical significance of exercise tolerance test (ETT) in the followup of Kawasaki disease(KD). Methods Among the 100 children diagnosed with KD in our hospital since 2003, select 54 cases who were suitable for the test and divide them into two groups according to the results of echocardiogram. There were 24 cases in coronaryarterylesion group and 30 in noncoronaryarterylesion group. Compare the parameters of EET in those two groups.ResultsAbnormal changes in ETT were detected in 26 out of 54 cases which accounted for 48.2%, and the coronaryarterylesion group shared 20 cases(83.3%) while the other group 6 cases(20%).The majority of changes in the coronaryarterylesion group represented as STT abnormality(16 cases, 66.7%), followed by decrease of tolerance dose(33.3%) and premature beat(20.8%). Comparing the 3 parameters between the two groups, the difference was statistically significant(P<0.05) while other parameters without statistical significance(P>0.05). ConclusionThe exercise tolerance test can be repeatable for the followup and diagnosis of KD patients which is one of the safe, feasible and noninvasive methods in evaluating coronary artery disease.

675 Related factors analysis of heart rate recovery after treadmill exercise test
Objective To explore the related factors which influence heart rate recovery after treadmill exercise test(TET). MethodsGive a retrospective analysis of 150 cases who were carried out TET in our hospital during May 2008May 2012 and were diagnosed with or excluded from coronary disease by undergoing coronary angiography(CAG). According to the occurrence of abnormality in heart rate recovery during TET, all patients were divided into two groups: abnormalheartraterecovery group(group A) and normalheartraterecovery group (group B), where the abnormality in heart rate recovery was defined if the recovery of heart rate the first minute after exercises was less than 18 time per minute. Various clinical observation indexes of these patients were analyzed.ResultsRelated factors of heart rate recovery after TET include being male, overweight, smoking, relatively fast basal heart rate, being complicated with coronary disease(especially multiple vessel disease), diabetes or anxiety.ConclusionIn the prevention and control of cardiovascular disease, it bears great clinical significance to control weight, quit smoking, slow down the basal heart rate, pay more attention to psychological intervention of mental disorders as well as function test of autonomic nerve of diabetes mellitus, and strengthen double heart treatment of coronary disease.

678 Analysis of electrocardiographic manifestations of acute pulmonary embolism in 18 cases
ObjectiveTo analyze the role that the changes of electrocardiogram(ECG)plays in the diagnosis of pulmonary embolism(PE).Methods  Retrospective observation method was used to analyze ECGs of 18 patients who had been diagnosed with PE. ResultsNormal ECGs were found in 3 cases and abnormal ones in 15 cases, where there were 7 cases (38.89%) of sinus tachycardia, 5 cases(27.78%) of SⅠQⅢTⅢ pattern, 5 cases(27.78%) of complete or incomplete right bundle branch block, 4 cases(22.22%) of right leaning of frontal plane, 3 cases(16.67%) of clockwise rotation, 11 cases(61.11%) of ST segment, 11 cases(61.11%) of T wave inversion, 2 cases(11.11%) of pulmonary P wave, 7 cases(38.89%) of terminal R wave in aVR lead as well as 4 cases(22.22%) of atrial arrhythmia.Conclusion Although the electrocardiographic diagnosis of PE is clinically nonspecific and nondiagnostic, characteristics of ECG have some value in suggesting the disease yet, especially the dynamic changes of ECG. Combing with clinical observation, it may help to make diagnosis and differential diagnosis, as well as reduce both of omission diagnostic rate and misdiagnosis rate.

681 Efficacy of amiodarone in the treatment of supraventricular tachycardia of acute coronary syndrome and acute heart failure: an analysis of 11 cases
Objective To evaluate the clinical efficacy of amiodarone in the treatment of patients with supraventricular tachycardia(SVT) of acute coronary syndrome(ACS) and acute heart failure(AHF). MethodsRetrospectively analyze the clinical data from 11 patients who were diagnosed with ACS and AHF with SVT. ResultsWhen conventional treatment proved invalid, intravenous amiodarone therapy was carried out. Among 11 patients, 9(81.8%) improved rapidly and cured at last and one died of cardiac shock as well as electric cardioversion was applied in another case. ConclusionThe possibility of ACS must be considered in patients with SVT and AHF. Amiodarone can be used effectively and safely to control SVT in patients with ACS and AHF.

684 Analysis of 5 cases with premature beat caused by belching
ObjectiveTo investigate the clinical manifestation,prognosis and intervention of the patients with premature beat caused by belching.Methods Select 5 patients with premature beat complicated with belching and make analysis clinically.ResultsAmong the 5 cases, there were 2 males and 3 females. All of them suffered discomfort in different levels, yet no evidence of severe arrhythmia which needed to be treated. During a followup of 13 41 months, adverse cardiac events didn′t happen to any case.ConclusionArrhythmia caused by belching is usually a benign process, the attack frequency of which associates with fatigue and mood. Therefore, antiarrhythmia therapy is unnecessary, yet psychological treatment could be helpful in eliminating arrhythmiarelated mental pressure. However, the attack frequency doesn′t decrease significantly.

686 WolffParkinsonWhite syndrome complicated with atrial fibrillation: a report of one case
Male, 41 years old, persistent palpitation associated with tachycardia. ECG: sinus P wave disappeared and ventricular rate was 193 bpm; QRS complex was basically identical in the shape while its duration varies in the width. It indicated the shortest RR interval was 200 ms. Combining the sinus ECG recorded in later period with the characteristic of identical shape in the beginning part of QRS complex, it′s not difficult to diagnose the disease as WolffParkinsonWhite syndrome complicated with atrial fibrillation.

688 An example of I° I atrioventricular block
Electrocardiogram (ECG) fails to record the dynamic change for short recording time. With wide application of dynamic electrocardiogram, some phenomena are found gradually which were not easily discovered previously. Thus, people pay more attention to these phenomena, including the change of PR interval of I° I atrioventricular block(AVB). It′s a must to give a  classification for I° AVB. Here we briefly introduce the classification of I° AVB as well as analyze some examples about it. It′s believed that I° I AVB will be restudied by ECGworkers with  the  application of dynamic electrocardiogram.
